Work is now underway to establish integrated home services for older people in Aarhus and ensure that the right service is provided by the right parties and at the right time. An agreement was confirmed in Aarhus today. Ina attendance were Inga Zealand, Minister of Social Affairs and Housing, Bragi Bjarnason, Mayor of Aarhus, and Diana Óskarsdóttir, CEO of the Southern Institute of Health, as well as numerous guests.

The agreement is between HSU and Árborg and is made on the basis of a development project on integrated home services, which is part of the government’s action plan, good to age.

This formal collaborative forum provided for the agreement will be the beginning of the further integrated home care residents of Aarhus. Emphasis is placed on individualized approaches and combining the aspects that the state takes care of on the one hand and the municipalities.

Collaboration was advertised with municipalities and health institutions on participation in the development projects, and Árborg and HSU were among those selected.

Connection consultant wins against social isolation

Part of the integration of the home service in Aarhus is to establish a special reception and assessment team and all requests for home services will be received for the team. Work will also be done to establish one service portal for all requests.

The interdisciplinary home rehabilitation team will also be established and most people who apply for home care will be in the service of the team to begin with. Home rehabilitation includes temporary self -help training, advice to enhance skills, activity and rescue in the activities of daily life as well as community participation.

This will be done, among other things, with the help of a connecting counselor, but his role is to counteract social isolation as well as being in good contact with local parties to make the most of the existing rocks in the community.

The rehabilitation team measures the skills and capabilities of individuals at the beginning of treatment and in the end. After three months of the team’s service, the need is reassessed and the maximum service time is six months. After that, integrated home services will take over if needed.

It is good that the older is a collaborative project of the Ministry of Social Affairs and Housing and the Ministry of Health.
