More Romanians could pay health contributions


The president of the National Health Insurance House (CNAS), Horațiu Moldovan, announced on Thursday, June 5, 2025, that the institution will propose a memorandum to make the expenses in the health system by the end of the month. One of the measures analyzed is the extension of the contribution base to the Unique National Health Insurance Fund (FNUASS), especially by including new categories of taxpayers.

Who could pay extra CASS

The statements were made at a conference organized by DC News Media Group and quoted by Agerpres. Moldovan said that it is not just about increasing the income, but also a matter of social equity. Among the categories concerned are certain large pensions-over 3,000-4,000 lei-and some groups of sewers that do not currently contribute directly to the health system. « There are certain categories that would deserve to be included not because they are constituted in additional income, but because it is socially correct, » explained the head of CNAS.

Why is extra money

According to CNAS data, the deficit of the National Health Fund reached almost 3 billion lei in the first quarter of 2025, despite the increase of revenues through the taxes introduced in 2023. Moldovan argues that the extension of the contribution base would allow the budget ceiling for cost-volume and cost-volume-results, used for the purchase of innovative medicines. « We could increase the budget limit allocated for cost-volume contracts, which is now 6 billion, but in reality we need 8 billion, » said CNAS president.

At the same time, the additional funds would allow to revise the financing of the basic package and a more sustainable payment of medical services, including at the level of family doctor, ambulatory and hospitals.

The reform of the hospital system

Moldovan also reported the problems related to the allocation of resources in hospitals, where the expenses with salaries exceed in many cases the payments for effective medical services. In his opinion, this system does not encourage performance. « Some hospitals that are not performing are quiet because they are provided with their staff, without managers being concerned about performance, attracting patients. »

In this regard, he proposed that the salary influences be included in the weighted case, in order to reflect the costs more correctly and to motivate the sanitary units to provide efficient services.

Possible changes in drug policy

The president of CNAS also spoke about the need to reform the mechanisms of pricing and repayment of the drugs. The measures would include a more rigorous assessment of clinical effectiveness in cost-volume contracts and a reconsideration of clawback and price-sharing mechanisms. « We are discussing mechanisms to quantify in cost-volume payments certain clinical efficacy elements, » Moldovan said.

Complementary insurance for risky lifestyle styles

Finally, the CNAS official also mentioned the possibility of introducing complementary insurance for people with lifestyles considered unhealthy, an idea to be analyzed in the next period.
